Education Professional Program Applications
Application and other materials that are part of the Professional Program application.

Form	Description
Early Childhood Ed Application	Professional program application for Early Childhood Education majors (Word file).
Elementary Ed Application	Professional program application for Elementary Education majors (Word file).
Secondary Ed Application	Professional program application for Secondary Education majors (Word file).
Special Ed Application	Professional program application for Special Education majors.
A.A.T. Student Application	Professional Program application for students transferring from a Maryland CC with an Associate of Arts in Teaching degree (Word file).
Criminal History Disclosure Form	Required form for the Professional Program application, this form needs to be notarized (PDF file)
Foundational Competencies/ Technical Standards Forms	Students must submit the Technical Standards Acknowledgement form, page 7 of the Tech Standards packet (Word File)
Undergrad Degree Audit Request	Student must submit a Degree Audit Request with their application (PDF File).

Exception to Policy Form	For student making a petition for an exception to policy (Word file).
Undergrad Degree Audit Request	Education major petition at least two audit request: 1) When applying to the professional program, 2) The semester prior to student teaching (PDF file).
